Weekend – On Call Care Manager, RPN

Reporting to the Director of Care, the On-Call Nurse is mainly responsible for providing on-call coverage as scheduled (on weekend & statutory holiday days), nursing support, coordinating & scheduling care for clients to ensure that they maintain their dignity, independence, and joyful living!
PLAN AND SCHEDULE CARE:
- Plan and schedule Caregivers with a particular focus on continuity and consistency of care for your Clients.
- Guide Caregivers to provide the highest quality of care through coaching, advice and support.
- Work with INTEGRAcare’s multidisciplinary team (Physiotherapists, massage therapists, doctors, dieticians, etc.) to provide comprehensive care for your Clients.

COMMUNICATE WITH CLIENTS & CAREGIVERS:
- Act as an expert nurse to all Clients and Caregivers by providing support and direction as needed
- Notify Clients and their Families of schedules and scheduling changes.
- Speak to Clients’ families in a sensitive manner when they have questions, or when an update on the health of the Client is appropriate
- Communicate & Liaise with Caregivers to ensure Caregiver availability.
- Match Caregivers with Clients based on personality, education, skills, Client’s scheduling requirements and Caregivers’ availability.
- Notify Caregivers of their schedules in a timely manner.
- Ensure that all shift offers and accepted shifts are logged in AlayaCare in a timely manner to maintain schedule integrity
- Ensure that scheduling of care and communication with Clients & Caregivers is clear and effective.
- Accurately document all new information regarding Clients and Caregivers as they occur
You will need to:
- Have a valid CNO Registration in good standing
- Be available to work Saturday & Sunday (8 am to 5 pm)
- Be available to work on Statutory holidays
- Have an ability to work independently
- Have a great attitude and willingness to work in a fast-paced environment
- Have strong Attention to Detail and desire to “Close the Loop”
- Possess good written and verbal communication skills
- Be passionate about providing our Clients and Caregivers with the highest quality care and support
- Be open to new learning opportunities, be coachable and dependable
Please note – this role will involve in-person paid training as per a mutually agreed schedule before the start date (mentioned below) at the 396 Moore office.

Job Type: Part – time
Schedule: Saturday – Sunday, 8am – 5pm & Statutory Holidays
Location: Remote (Work from home)
Pay: $30.00/ hour (Stat holiday pay to be applicable when working on stat holidays)
